  • Acknowledgment that the Scholarship Selection Committee will examine your IU academic transcript in considering your application for a scholarship.
  • Your intent to complete a minimum of 12 graded credit hours both semesters of academic year 2008-09. Please note: Except in the case of extraordinary circumstances, students who are awarded a scholarship but who do not fulfill the full-time requirement may be asked to repay any scholarship monies posted to their bursar accounts.
  • Approval for use of your name in any media for publicity purposes.

Recommendations  Print Recommendation Forms
Applicants must submit two letters of support, using the form provided with this application. You will need to print one copy for each person submitting a recommendation on your behalf.

One recommendation should be from a faculty member in the life sciences area who has taught you at IU, preferably one who can comment on your participation in class discussion or laboratory work. The second letter should come from another IU faculty member or AI (from either the College of Arts and Sciences or the Kelley School of Business) who can comment on your strengths and ability to succeed in a program like the Life Sciences Scholars Program.

Print a copy for each person writing a recommendation on your behalf. Fill out your name, and complete the waiver portion of the form. Once the person writing in support of your application has completed the recommendation the forms should be sealed in an envelope and signed across the seal. As the applicant, it is your responsibility to ensure that the recommendation is received in Kirkwood Hall 012 before the application deadline: 4:00 p.m. on Friday, March 7th, 2008.
